The suggestion should be of a health care professional, organisation, or support network that has provided care you would recommend to any other individual and family whose lives are impacted by Childhood-onset Heart Disease (CoHD).

After submitting the details a staff member will be in contact with the person or organisation you suggested, to verify the details and confirm their consent to be listed.
